Our “Directory to Doorstep” model enables you to connect to support that matters to you and to support Social Prescribing on island.

We have just attended the International Social Prescribing Network Conference in London – it was inspirational! We are up to date with the latest research and thinking on the implementation and practice of social prescribing around the world and we look forward to working with our local partners in health and care in the Bailiwick to ensure its success locally.

What is Social Prescribing?
‘Social Prescribing is a means of enabling GPs and other frontline healthcare professionals to refer patients to a link worker – to provide them with a face to face conversation during which they can learn about the possibilities and design their own personalised solutions, i.e. ‘co-produce’ their ‘social prescription’- so that people with social, emotional or practical needs are empowered to find solutions which will improve their health and wellbeing, often using services provided by the voluntary and community sector.’ – The Social Prescribing Network who will be able to promote their offer on our online Directory.
