Health Connections celebrated their 1st anniversary recently at Les Cotils with guest Speaker Dr Roger Allsopp. His talk “The positives of an Ageing Demographic- from Retirement to renaissment” was exceptionally well received by a large and diverse audience, which included their patrons Dame Mary Perkins and Lady Corder.

The address on the evening by HC CEO Bella Farrell, focused on their progress.

By end of Q3 Health Connections had provided 886 hours of support to the community!

Their online directory has over 300 directory entries and they have trained 69 Community Connectors to signpost people to this directory for support.

Their Voluntary Community Transport service drivers had delivered 3160 journeys by end of Q3 making it the busiest year to date with some weeks servicing over 115 journeys. They currently. supports over 200 people with unmet transport needs. A fully accessible minibus gifted by Age Concern is now also part of their offer.

Their off island hospital packs can now be downloaded from their website. They have distributed over 400 off island hospital packs this year so far! The service is keen to provide more face to face support to those people and their families who are going off island for treatment and care.

Health Connections is now a key partner in the Partnership of Purpose Social Prescribing project. Thanks to funding from the Guernsey Community Foundation they have employed a Social Prescribing Commissioning Manger who will work alongside the public health team at HSC.

Their link worker service refers people to support that matters to them in the community and promotes all the opportunities for improved health and wellbeing offered throughout the islands.

HC is now well on its way to ensuring local people are connected to support and information that enables them to live a happier, healthier and more fulfilling lives.
