Our community support directory strengthens connections by improving awareness and access to statutory, private, third-sector, and community resources.

How do we help?

We're working hard to create a Bailiwick where everyone can find and connect to the support they need for their health and wellbeing. Learn more about the impact we have in the community below.

Community Directory

  • Over 600 community assets listed.

  • Over 35,000 annual viewers.

Outcomes:

  • Increased access to support: Facilitate quicker connections with appropriate services.​

  • Empowerment and independence: Enable individuals to manage their health proactively.​

  • Community engagement: Promote social connectivity and reduce isolation.​

Signposting Support

  • Over 3,000 signposting conversations per year.

 

Outcomes:​

  • Enhanced health and wellbeing for Bailiwick residents by promoting self-care, connecting people with the support they need and fostering resilience and independence.​​

  • Increased access to early intervention and preventative support, helping individuals through key life transitions, such as diagnosis or bereavement - leading to better health outcomes, reduced isolation and reduced acute healthcare costs.​

Talking Cafés

  • 5+ Talking Cafes per week.​

  • Over 360 hours of signposting support per year.​

Outcomes:

  • Reduced isolation: These valuable social connection opportunities actively reduce loneliness and foster a sense of community and enable individuals to share experiences and build meaningful relationships.​

  • Improved access to information: This personalised interaction helps individuals navigate complex services.​

Transport Service

  • 7,500+ journeys delivered per annum​

  • 500+ journeys delivered monthly​

  • 350+ registered passengers​

Outcomes:

  • Improved access to health and wellbeing support and opportunities for social connection.

  • Increased equality and independence and a reduction in isolation and loneliness.​

  • Reduced missed appointments and support early intervention and prevention of ill health.

Off-Island Support

  • We supported 300 individuals with Off-Island care information in 2024.

Outcomes:

  • People going off island for treatment and care feel supported and informed.​

  • One-to-one support, digital and paper hospital packs help guide people and their families through this experience.​

  • Connecting people to information and support that matters to them. ​

Charity Shop

  • Achieving 60% self-sufficiency.​

Outcomes:

  • Revenue generation: Our shops have contributed to achieving financial self-sufficiency.​

  • Community engagement: Volunteers and customers actively participate in creating a connected community.​
