Lead Community Connector

15-25 hours per week

Key Responsibilities

The duties of this role relate to developing and supporting a network of trained Community Connectors.
This role supports that of the charities Lead Health Connector and the Directory Co-Ordinator and may
include are not limited to delivering Community Connector projects, in line with Health Connections (HC)
agreed priorities as outlined in its Business Plan:

  • identifying, training and supporting a network of Community Connectors who signpost their friends,
    family, colleagues and neighbours to services, support and activities listed on the Health Connections
    (HC) online Directory
  • developing and delivering the Talking Cafe project and supervising Community Connectors who
    volunteer in the Talking Cafes,
  • organising regular networking events for Community Connectors and directory partners to promote
    and publicise the support in the community
  • working with the Directory co-ordinator to continue to map the assets in the community and proactively
    seeking new community groups and activities that support health and wellbeing
  • using creative and innovative ways to engage directory partners in our charity shop, CONNECT
    community hub and through our network events
  • developing a Community Connector phone signposting service to support the lead Health Connector
  • monitor and evaluate all work through Health Connections monitoring procedures
  • implementation of Health Connections policies in all aspects of the work
  • keeping accurate records of Community Connectors; partnerships and activities to assist in the CEO’s
    reporting to the board and provide regular written/spoken reports and analysis of work to the CEO as
    required.
  • attending training and comply with Information Governance, data protection and confidentiality
    policies procedures and code of conduct
  • be responsible for the health and safety of any volunteers and trainee CC’s
  • actively participate in supervision
  • demonstrate a commitment to continuing professional development and to attend mandatory training.
  • carry out such other tasks as are required to meet the aims and objectives of Health Connections and contribute to the smooth running of its office.

Qualifications and Experience

Experience of supporting or working with people and volunteers in a social or caring role either voluntary
or paid is beneficial

  • A suitable training qualification for teaching adults such as AET or Certificate in Education and/or
    experience of facilitating training sessions is preferred but not essential
  • Suitable level academic qualification to at least level 3 or above and/ or administrative experience is
    preferred
  • An understanding of Safeguarding vulnerable adults and data protection (further training provided).
  • Experience in organizing and hosting small events is preferred but not essential
  • Excellent organisation, planning and prioritsation skills
